I agree with that it is a song of affection and sympathy, which tells a person's story without imparting judgment or cliché. So, that could imply apathy or lack of hope, meaning continued use due to loss of self-respect or simply bc many junkies often vocalize a desire to quit, but lose that drive once dope sickness and/or emotional anguish kick in. And while the lyrics are clear, they still refrain from reaching cliché. It refrains from preachy, anti-heroin schtick, but also steers clear from glamorizing Judy's lifestyle as a junkie. That's what I respect so much about this song and what always drew me to it. Bingo! While the song itself is pretty straightforward (aside from the disarmingly poppy melody/vocal harmonies), I totally agree withdrawn you that there is a strongly sympathetic tone to the lyrics.
